Forms & Eligibility

Athletics Forms Compliance

FORMS DUE!
There are three forms required of any Monte Cassino Middle School student (grades 5 - 8) wishing to participate in Monte Cassino sponsored sports.  In order for your child to be eligible to participate in a school-sponsored sport, the following forms must be completed and submitted.
  • Pre-Participation Physical Evaluation - Must be uploaded during registration
  • Sudden Cardiac Arrest Acknowledgement Form - Complete during registration.
  • Monte Cassino Athletics Code of Conduct - Complete during registration.
If these forms are not completed and submitted, prior to the start of their Monte Cassino team's athletic season, your child will be ineligible.

Ineligible players may continue to practice but they will NOT be able to participate in practices, scrimmages, games and/or tournaments with his/her team.  Students determined ineligible, will remain ineligible until all three forms have been completed and submitted.

Athletic Academic Eligibility Policy

Athletic Eligibility Policy

All Middle School Students (Grades 5-8) must maintain a passing grade (above a “D”) in each class during their Monte Cassino team’s athletic season to remain eligible for the participation in athletic events. Grade checks will occur every Friday during the year and students are determined to be on probation or ineligible.

Academic Probation

After each grade check, students with a grade of “D” or “F” in any class will be placed on academic probation. Students are still eligible to participate in practices and games, but they have one week to improve their grade before becoming ineligible. Students, parents, and coaches will be notified of the probationary status, along with steps to help the
student improve.
Students with a “D” or “F” in any class are required to attend Tutorial sessions for that class, Monday through Thursday, from 3:05 PM to 3:30 PM.

Ineligible Players

If the grade is not above an “D” by the following grade check, the student will
become immediately ineligible to participate in practices and games until the grade improves. Tutorial attendance is mandatory to regain eligibility and until the grade has improved.

Tutorial Requirements

All students attending Tutorials must submit a Tutorial Sign-In Form signed by the teacher to the Athletic Department each Friday to confirm attendance. Teachers are kindly asked to do the following:
  • Grade Updates: If a student’s grade improves to a passing level before the next grade check, please email the Athletic Department with this update.
  • Comments Section: When students attend Tutorial, please note their updated grade in the “Comments” section of the Tutorial Sign-In Form if it has improved to passing.
This policy ensures that student-athletes remain focused on their academic responsibilities while continuing to benefit from the physical, mental, and social development that athletics provide.

  • Tutorial

    All players with a grade of “D” or below, in any class, will be required to attend Tutorial for that particular class, Monday through Thursday 3:05 pm- 3:30 pm, until the grade is above a “D.” Students with a “D” who remain above an “F” will remain eligible for games and practices. However, tutorial is mandatory even when games and/or practices occur during Tutorial time. Tutorial students are required to turn in a Tutorial Sign-in Form, signed by the teacher, to the Director of Athletics each Friday.
  • Disruptive Behavior

    Behavioral issues which occur on campus during and after school may also be grounds for ineligibility or other athletic penalties based on the discretion of the Middle School Principal and the Director of Athletics.
  • Effects on Participation

    Missed practices due to academic or behavioral infractions may result in reduced playing time and is managed at the Coach’s discretion.
